You could build a simple form for each course with one of the many form builder plugins that exist out there. Would that work?
There are a lot of plugins for booking systems like hotels and such, but this is a bit different. Thre are some premium solutions that could work. Check out this plugin and you could ask the author if it would be possible for you case.
