I had this with subdir multisites and no permalink structure set. It was adding index.php at the end of the site url and then tacking on the ajax url. Setting the permalink structure to something other than default "fixed" it, but so did commenting out lines 61-66 (the if statement in _add_index_dot_php_to_url) in wordpress_routing/adapter.wordpress_router.php
