madelynn on "Searching for plugin to use as a question map"

I'm looking for a plugin that will create a question map with answers. I want it to work similar to this (ExpressionEngine site): http://www.firstcoastymca.org/programs/learn_to_swim

This client is a church so the questions would be similar to the following:
Do you prefer a traditional service or a contemporary service?
Do you have children?
Do you prefer to worship on Sunday or on a weekday?

Based on the users' answers it will display the correct service options for them.

Any ideas or direction? I'm lost at even knowing what terms to search for!
